Forms for Health Office:

1Physical Exam and Immunizations Form. Physical and Immunizations: student needs updated PE for sports every 2 years or when the student receives any new vaccines

2Permission for exchange of medical information. Permission form to share medical information: Needed to obtain medical records from previous school or any physicians

3. Student Emergency Information FormEmergency form: Student health information/Emergency contacts/Permission to treat and transport

4.NH State Immunization Requirements for 2009/2010: Student must comply with state requirements each year

5.Medication Orders and Parent Permission Form: Must have on file in order to administer medications in school
