New Student Registration
2024-2025 Ellis School Registration Form
As part of the registration process please submit the following documents to the Ellis School main office along with the registration form.
Copy of student's birth certificate
Copy of student's most recent immunization record
Copy of student's most recent physical examination record
Copy of parent/guardian ID
Ellis School Residency Verification - please provide one of the the items listed under accepted forms of residency.
Items accepted to prove residency
Copy of a fully executed lease, with the name and phone number of the landlord
Copy of a fully executed closing statement/warranty deed
Telephone (landline), electric bill, or cable bill
Envelopes witha yellow forwarding postal sticker
Billing or mailing address from current doctor's bill, bank statement, or payroll check
Notarized letter from the town hall stating that you are a Fremont resident. A copy of one of the accepted proofs above must be attached to prove residency of the resident writing the letter.

Water/sewer bill
Voter registration
Vehicle registration
Driver's license
Post office box
